Patients Rights and Responsibilities

RIGHTS AND RESPONSIBILITES

OF THE PATIENTS & PARENTS/CAREGIVERS

1. Patients and their families have the right to be treated with respect and dignity and receive treatment in a safe environment.

2. Patients and their families have the right to be treated by clinicians who adhere to their professional codes of ethics.

3. Patients and their families have the right to privacy and confidentiality of information. The Detroit Institute for Children (The DIC) adheres to Federal and State confidentiality laws.

4. Patients and their families have the right to freedom from abuse, financial or other exploitation, retaliation, humiliation and neglect.

5. Patients and their families have the right to information about treatment, treatment recommendations and the possible outcomes should those recommendations not be followed. They have the right to get this information in enough time to make an informed decision.

6. Patients and their families have the right to refuse service, to withdraw consent and discontinue treatment at any time. They may refuse or express their choice regarding service delivery, release of information, concurrent services, or the composition of the service delivery team. If consent for treatment is withdrawn, a written request is required.

7. Patients and their families have the right to access to or referral to legal entities for appropriate representation.

8. Patients and their families have the right to access to self-help and advocacy support services.

9. Patients and their families have the right to express any concerns or complaints about the services received at The DIC or any alleged infringement of these rights. All complaints will be investigated and resolved.

10. Patients and their families have the right and responsibility to help plan treatment, to carry out the treatment plan and goals to the best of their ability, and to evaluate the services received.

11. Patients and their families have the responsibility to treat staff and other patients with respect and dignity.

12. Patients and their families have the responsibility to be on time for appointments. If they are late, the appointment may have to be rescheduled.

13. Patients and their families have the responsibility to attend all scheduled sessions or call to cancel a scheduled appointment 24 hours in advance. Failure to do so may result in discharge from services.

14. Patients and their families have the responsibility for any fees that may be charged at the time of service.

To care for children with physical or developmental disabilities, neurological or behavioral special needs in a way that is positive, passionate, integrated and family-centered, so that the children can maximize their potential within their families and the community.
